Provo, UT – Aug 1st, 2024 –Orthopedic surgeon Dr. Scott Evans is leaving Revere Health due to his relocation from Utah to Washington State this year. He is scheduling clinic appointments through August 14.

Drs. Frazier and Johnson have agreed to provide follow-up care for any of Dr. Evan’s patients with ongoing needs, such as post-operative care, medical questions, or medication requests.
