We believe insurance should not be a barrier to quality care. Therefore, Revere Health participates with most plans operating in the state of Utah either directly or indirectly, including SelectHealth. We contract with major insurance organizations (payers) as well as local and national networks that have hundreds of smaller payers. We also work with third-party administrators and employer groups and organizations that access these provider networks.
With few exceptions, Revere Health is likely going to be considered “in-network” with most insurances. The following is a summary of insurance plans accepted by Revere Health.
Please note this does not necessarily represent a complete list of all plans. There are exceptions and circumstances where participation may vary based on geographic location, provider credentialing, as well as other factors.
Revere Health providers participate on American Health Advantage, which is an Institutional Special Needs Plan contracted with Medicare. The plan is specially designed for eligible Medicare beneficiaries living in a long-term care nursing home. Currently, American Health Advantage is covering residents of long-term nursing homes in Box Elder, Salt Lake, Utah and Weber counties.
Revere Health providers participate on all BridgeSpan plans offered on the Healthcare Exchange Marketplace in Utah. BridgeSpan is a Regence BCBS company and falls under our relationship with Regence BCBS.
As of January 1, 2023, Bright Health no longer operates any plans in the state of Utah.

Revere Health providers participate on the Cigna Medicare Advantage plan operated by Cigna HealthSpring that was rolled out January 1, 2021. The American Fork Surgery Center is not currently included in the Cigna HealthSpring network.

Revere Health providers participate on all plans offered in the state of Arizona by Health Choice Arizona (Southern Utah Providers Only). These plans (Managed Medicaid Arizona and Medicare Advantage plans) are currently administered by Anthem BCBS of Arizona.
Revere Health providers participate on this network which is offered to employer groups through the state. Starting 2022, this network is administered by Motiv Health. Revere Health is only currently participating on the Health Utah Network which is the network that Motiv Health uses for its provider network.
Revere Health providers participate on most plans offered in the state of Utah. This includes most ancillary services, including the Surgery Center and St. George Imaging Center.
Revere Health providers rendering services in our Mesquite, Nevada location are participating with Intermountain Healthcare Nevada’s SelectHealth Advantage plan. This is the SelectHealth Medicare Advantage plan in Nevada that is administered by Intermountain Healthcare Nevada (formerly HealthCare Partners of Nevada). Please note that Intermountain Healthcare Nevada administers other plans in Nevada where Revere Health is non-participating. Services rendered in Utah would fall under our SelectHealth contract in Utah.
Revere Health providers in the Southern Utah region participate with Injury Care Solutions including the Imaging Center in St. George.
Revere Health providers participate with Liberty HealthShare as of February 2017. Liberty HealthShare is a health cost sharing organization (501(c)3), that provides their members with a non-insurance sharing of healthcare expenses.
Revere Health providers participate with Mercy Care in the Southern Utah region. Mercy Care is a managed Medicaid plan in Arizona and their members can come across the border for care from Revere Health providers.

Revere Health has participated with the Health Utah Network for many years. The Network was acquired by Motiv Health and they use this provider network for all of their current products in the state of Utah.
Revere Health providers participate on all networks operating in the state of Utah under the MultiPlan umbrella.
Revere Health providers located in St. George, Kanab and Mesquite, Nevada are participating on this plan. P3 administers the SelectHealth Medicare Advantage plan in the state of Nevada. Patients living in Nevada on this plan can access care with Revere Health providers in Mesquite and in Utah with our Southern Utah providers.
Revere Health providers throughout the state are participating on the Provider Network of American PPO Network (PNOA-PPO). This is a network of self-funded groups all over the country and the state of Utah. PNOA uses the Intermountain Healthcare system for hospital and skilled nursing facilities.
Only Revere Health’s Imaging Centers and Radiologists are participating in the One Call Care Network. Imaging Centers include American Fork, Provo, Salem and St. George.
Revere Health providers participate on most plans offered in the state of Utah. This Network operates under the United Health Group umbrella and administers the UHC – AARP Medicare Advantage Plans in the state or Utah.

Samera Health is TPA formerly known as Benefit Administrative Solutions (BAS). It initially started as Town & Country Provider Network, but has since rebranded to Samera Health. All Revere Health providers and locations are participating on this network.

Revere Health is contracted on all lines of business offered by SelectHealth, but individual participation and plan offerings may vary by provider, geographic factors, hospital privileges and other special circumstances. Additionally, there are certain exclusions related to Revere Health’s ancillary services, such as American Fork Surgery Center and the Coral Desert Imaging Center in St. George. For Select Share, all clinic providers and services are considered in network.

Revere Health has a group contract and providers are routinely credentialed with Health Net Federal Services which administers Tricare for active military and their dependents. Health Net Federal Services took over administration of Tricare starting 1/1/18. United Health Care will continue to runout claims for Tricare for services rendered prior to 1/1/18.
Revere Health providers participate on most plans offered in the state of Utah. Tricare for Life is a secondary plan for Medicare beneficiaries – any provider enrolled with Medicare will be able to accept Tricare for Life.
Revere Health is participating with the VA Community Care Network administered by TriWest in the West Region (VA Region 4). The CCN combines and consolidates programs formerly known as VA Choice and VA Patient-Centered Primary Care programs.
