On August 1, 2018, Revere Health transitioned to a new billing software. Statements from the old billing system for a balanced owed before August 1 will have CUM (Central Utah Medical) in front of the account number. The new statements do not have alpha characters in front of the account number.

Note: You will need to include your account number (Patient ID/Chart #) when making a payment. On some statements, prior to May 1, it will be listed as Account Number. On more recent statements it will be listed as Patient ID/Chart #. See examples below.
