Beginning on January 1, 2024, the Central Utah Clinic PC dba Revere Health ACO REACH has been renamed the Revere Health Collaborative Network ACO REACH, reflecting the addition of Utah-based Canyon View Medical into Revere Health’s ACO REACH model. ACO REACH stands for Accountable Care Organization (ACO) Realizing Equity, Access, and Community Health (REACH).
The ACO REACH MODEL is comprised of doctors and healthcare providers who have come together voluntarily to provide coordinated, high-quality care at lower costs to their Original Medicare patients. ACO REACH participants are patient-centered organizations where the patient and providers are true partners in care decisions. Participating patients will see no change in their Original Medicare benefits and will keep their freedom to see any Medicare provider.
Included in the ACO REACH MODEL are strong patient protections to ensure that patients have access to and receive high-quality care. Like other Medicare ACO initiatives, this Model will be evaluated on its ability to deliver better care for individuals, better health for populations, and lower growth in expenditures.
All members of Revere Health will work together to see that you get the right care at the right time. We will protect your medical records and privacy. We will work to reduce duplicate tests and duplicate paperwork. We will help everyone work smarter to give you better care. We will coordinate your care according to your medical needs and choices.

Skilled Nursing Facility: Advance Healthcare of Salem
In an effort to improve coordination of care and create programs that will result in achieving Revere Health’s “Your Health Above All Else” experience, while improving care for the Medicare Fee-For-Service beneficiaries we serve, better health for the populations we care for and doing so in a financially sustainable manner, Revere Health and its participating providers, working in collaboration with RH, formed the joint venture, Advance Healthcare of Salem.

The Secretary of the U.S. Department of Health and Human Services has provided waivers of certain federal fraud and abuse laws that otherwise limit innovation by REACH’s participating in the Accountable Care Organization (ACO) – REACH Model (see HHS, Notice of Amended Waivers of Certain Fraud and Abuse Laws in Connection with the ACO – REACH Model, published December 29, 2016). Pursuant to that notice, Revere Health seeks protection under the ACO REACH Participation Waiver for the following arrangements set forth below.
