Jason Gray grew up in Utah County and graduated from Brigham Young University. He married his high school sweetheart, and went to PA school at A. T. Still University in Mesa, Arizona. While in Arizona, Jason spent time working in hand surgery with the Arizona Diamondbacks and in urgent care. 

He enjoys being able to make a difference in the lives of his patients and see their quality of life improve as they respond to treatments.

Outside the clinic, Jason enjoys sports, exercise and spending time with his three children.

Survey Details

CAHPS surveys are funded and overseen by the Agency for Healthcare Research and Quality (AHRQ) within the United States Department of Health and Human Services (HHS).

Patients receive the nine-question survey via email or phone call, and we publish provider ratings on Revere Health's website once he or she has at least 30 completed surveys.
