Kavya Rao, MD, Leaving Revere Health

Neurologist Kavya Rao, MD, will be leaving Revere Health on June 4th, 2025. She has chosen to accept a remote EEG reading position to better attend to the current needs of her family.

To meet the continued needs of your child, Dr. Rao recommends the options listed below. She suggests that patients be proactive in having their primary care provider send a referral to their clinic of choice as soon as possible due to the lengthy wait lists for neurology providers in Utah.

OPTIONS FOR CHILD NEUROLOGY CARE

If your child has genetic epilepsy or complex epilepsy (your child is on two medications or more, or has failed two medications), please specify this in the appointment request. If your child has a VNS device, please have your primary care provider mention this in the referral.

If your child is a teenager, you can call one of the following practices and ask if they have age minimums. You may be able to get an appointment sooner with one of these providers than you can with a child neurologist office.

TRANSFERRING RECORDS

If you would like your child’s records transferred to another provider, please call Revere Health’s Records Department at 801-429-8062 or click here to request your records online. Any hospital records would have to be requested from the hospital your child was seen. Dr. Rao recommends that you obtain your records after your child’s last appointment so that they include my latest notes.
