My Dear Patients and Friends,

It has been my great pleasure to care for you over the last several years, but I am writing to let you know that May 19, 2023, will be my last day at Revere Health.

I want to thank you for choosing me as your trusted care provider and hope you will continue to come to our clinic and visit any one of my talented colleagues for your nephrology care needs. Your medical records will remain at Revere Health, but if you choose to seek care elsewhere, you may fill out an authorization form to transfer your records to a new provider. Call (801) 429-8062 for assistance. You can also use our patient portal, FollowMyHealth, to download a copy of your records.

I have truly enjoyed caring for you and getting to know you. I thank you for the positive impact you have made on my life—I hope I was able to do the same for you.
