Lance Allen, MD, Leaving Revere Health

Lance Allen, MD, is moving out of the Kanab area and will therefore be leaving Revere Health. His last day with Revere Health will be June 6, 2025.

We hope you will continue to come to Revere Health for your Family Medicine needs. Doctors Eric Root and Jonathan Bowman are accepting new patients and are well-equipped to handle pediatric, geriatric, and Adult Medicine needs.

The clinic has hired a new Nurse Practitioner and is actively looking for another physician to serve in Dr. Allen’s place. Please be patient during this time of transition.

Your medical records will remain at Revere Health, but if you choose to seek care elsewhere, you may fill out an authorization form to transfer your records to a new provider. You can request your medical records on our website at reverehealth.com, through the FollowMyHealth patient portal, or call 801.429.8062 for assistance.
