Dear Valued Patient,

It is with mixed feelings that I would like to inform you of my departure from Revere Health. I will continue seeing patients in the office and via telehealth appointments until March 28, 2023.

I have had the privilege of caring for you and your family, along with many others in the Washington County area, for several years. It has been my honor to provide your care. I have had great joy in being of service to you.

It has also been a privilege to work together with so many other good physicians, therapists, and staff at Revere Health who are also committed to high standards of medical and mental health care. Darrin Brandt and Rafael Sanchez will continue to provide excellent care for the people in our community after my departure. They are exceptional LCSWs that offer dedicated assistance for every client’s needs. You may make an appointment with them to continue your care. To schedule an in-office or telehealth visit with Rafael Sanchez in the St. George area, you may call the office at (435) 215-0344. To schedule with Darrin Brandt for a telehealth visit, please call his office at (385) 203-2750.

We hope to continue to provide for your mental health care needs at our St. George Behavioral Health office. However, if you wish to transfer your care to another provider outside of Revere Health, we can assist in transferring records. Once again, I would like to thank you for allowing me to serve you in your mental health care needs.
