REACH Termination Notice

Revere Health has been an accredited member of the Medicare Accountable Care Organization (ACO) program since 2012. A Medicare ACO is a group of doctors, hospitals, and other healthcare providers who come together voluntarily to provide coordinated, high-quality care to people with Medicare.

The goal of an ACO is to ensure you get the right care at the right time—while avoiding unnecessary services and reducing medical errors. Over the last 13 years, our participation in this program has saved our patients over $80 million dollars. As a patient, you don’t need to do anything to be a part of our ACO–your providers take care of that behind the scenes.

Since 2023, Revere Health has participated in one of the ACO programs called ACO REACH, and our particular ACO REACH is known as the Revere Health Collaborative Network. As of December 31, 2025, we will no longer participate in this particular ACO program. You are receiving this notice because your doctor is currently a participating provider of the Revere Health Collaborative Network.

While in the ACO REACH Model, Medicare allowed us to offer patients additional support— sometimes called “benefit enhancements.” These included things like extra follow-up care after leaving the hospital, certain co-pay assistance, or other special services. Because these were available only during our participation in the REACH program, Medicare will no longer cover them after April 1, 2026. If you choose to receive these services outside of the program, you may be responsible for payment.

Please be assured that this change will not affect your regular Medicare benefits, coverage, or the provider you choose. You will have access to the same services you receive today under Medicare.

Beginning in 2026, we will begin participating in a different ACO program called Medicare Shared Savings Program (MSSP). This means that our providers will continue to be committed to the same goals of every ACO—providing the highest possible quality care in the most affordable way possible. You will receive a notification of our participation in this program by May 31, 2026.
