Medicare Wellness Visits

Medicare Wellness Visits are offered at Revere Health to help you stay proactive about your health as you age. These annual visits are designed to create or update a personalized prevention plan that addresses your unique health needs. Medicare covers one wellness visit each year for patients who have had Part B coverage for over 12 months. The visit is different from a traditional physical exam and focuses on preventive care and health maintenance.

What to Expect During a Medicare Wellness Visit
During your wellness visit, our providers will:

Review your personal and family medical history
Discuss current medications
Measure height, weight, and blood pressure
Provide a Health Risk Assessment, which is a questionnaire to identify potential health risks
Offer personalized health advice and set health goals
Perform a cognitive assessment to check for signs of memory issues or dementia
Create a schedule for recommended screenings and immunizations based on your health needs
If needed, the visit can also include discussions about advance care planning, like living wills and treatment preferences, to ensure your long-term healthcare goals are met.

Why Schedule a Medicare Wellness Visit?
This appointment helps prevent disease and manage chronic conditions by allowing your provider to identify health risks early. It’s an opportunity to talk about your overall wellness and receive guidance on nutrition, physical activity, and lifestyle adjustments to support healthy aging. Medicare covers the cost of the annual wellness visit as long as your provider accepts Medicare, so there’s no additional cost to you unless other non-preventive services are provided during the same visit.

Staying proactive about your health helps improve your quality of life and keeps you informed about how to manage your health long-term. Contact Revere Health to schedule your Medicare Wellness Visit today and take the next step toward maintaining a healthy lifestyle.
