Preventative Health and Wellness
Our Preventative Health & Wellness clinic, based in Provo, serves as a central hub for all of Revere Health. While our office is located in Provo, we proudly support providers and patients across every Revere Health clinic. We focus on proactive, team-based care that empowers patients to manage chronic conditions, close care gaps, and prevent avoidable hospital and ER visits. Our nurses and care coordinators partner with clinics throughout the organization to deliver services such as Chronic Care Management, Remote Patient Monitoring, Transitional Care Management, and preventative outreach. By working alongside clinic teams, we enhance patient support between visits, improve quality outcomes, and reduce the overall cost of care.
