Chaz Davis, FNP-C


Bio	Chaz Davis, FNP-C, completed his undergraduate training at the University of Utah and his Master of Science in Nursing at Indiana State University. He has experience providing evidence-based treatment in both family practice and urgent care settings. He has also worked with the Huntsman Cancer Institute to develop a home-based, cancer-related treatment model. In this capacity, he has been published in scientific journals and has developed a passion for oncology. Chaz is board-certified by the American Academy of Nurse Practitioners (AANP). Chaz brings an inquisitive and thorough approach to patient care. He values patient preferences when developing care plans. He strives for strong collaboration with his patients and other members of the care team to ensure the best possible outcomes. When not in the office, Chaz enjoys being outside. He loves mountain biking, camping, hiking, and exploring the outdoors. He enjoys being with his family and loves to cheer for his children at sporting and school events.
