Changing Your Claims Data Sharing Preferences with CMS

The Centers for Medicare and Medicaid Services (CMS) shares claims data with Revere Health, a Next Generation Accountable Care Organization (NGACO), for beneficiaries in the NGACO program. Claims data is shared for the purposes of care coordination and quality improvement.

If you are a beneficiary in Revere Health’s NGACO program and you would like to know more about claims data sharing, or modify your preferences regarding claims data sharing, please contact Medicare directly by calling 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048.

Please note that even if a Next Generation Beneficiary elects to decline claims data sharing, CMS may still engage in certain limited data sharing for quality improvement purposes.

If you have previously contacted CMS to opt out of sharing your claims data and you wish to reverse that preference or make any other changes regarding your preferences, you may do so by calling 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048.

Substance Use Disorder Data Sharing Opt-In

As a Next Generation Beneficiary, you may elect to allow Revere Health to receive beneficiary-identifiable data regarding your utilization of substance use disorder services. This information will NOT be shared with Revere Health unless you specifically request to opt-in.

If you would like to opt in to having substance use disorder data shared, please submit this CMS-approved substance use disorder form to Revere Health (the ACO) using the email patientconcerns@reverehealth.com. Revere Health will promptly send the opt-in form to CMS.
