Dear Valued Patient,

It has been my great pleasure to care for you and your family, but I have decided to leave this practice at the end of March 2023. As a result, our North Orem Family Medicine office at 700 West 800 North will be closing. The Orem Internal Medicine office at the same location will remain open.

Zachary Reese, PA, will be staying with Revere Health and moving to Orem Internal Medicine. Emily Robbins, NP, is also leaving Revere Health.

Despite this office closure, Revere Health remains committed to providing you and your family with quality healthcare. We have several other locations in Orem staffed with highly qualified providers ready to meet your needs:

Your medical records will remain at Revere Health, but if you choose to seek care outside of Revere Health, you may fill out an authorization form to transfer your records to a new provider. You can also use our FollowMyHealth portal to download a copy of your records. Call (801) 429-8062 for assistance.

Thank you for trusting us with your healthcare. We apologize for any inconvenience this office closure causes, but we are more than happy to continue caring for you in one of our other Orem locations.
