It has been my great pleasure to care for you over the last year, but I have decided to leave this practice. As of December 2, 2022, I will be leaving Revere Health.

I want to thank you for choosing me as your trusted care provider and hope you will continue to come to our clinic and visit any one of my talented colleagues for your Cardiology care needs. Your medical records will remain at Revere Health, but if you choose to seek care elsewhere, you may fill out an authorization form to transfer your records to a new provider.
