Attached is our paperwork for new patients. Please fill this paperwork out completely and email your paperwork to SouthernUtahSpineRehabilitation@reverehealth.com or bring a printed copy with you to your appointment.
We ask that you arrive at least 15 minutes early for your appointment to allow us time to process your paperwork, check you in, and obtain a copy of your insurance information. If you are not able to finish your paperwork, please arrive even earlier so that you can complete it at our office before your scheduled appointment time.
