Mandy Lemmon, APRN, FNP-C, completed her Associates Degree in Nursing from Weber State University and then received her Bachelor of Science Degree in Nursing at Western Governors University. Later, she pursued a Master’s degree in Nursing, specializing in family medicine, from Keiser University. She has been recognized for honors in nursing, received multiple nominations for the DAISY Nursing award and was a recipient of the Western Governors University Excellence award. Mandy is board certified with the American Association of Nurse Practitioners. She is a current member of the American Nurses Association and the American Association of Nurse Practitioners.
Mandy initially chose the healthcare field because she loves making a difference in other people’s lives. She chose to specialize in family medicine because it allows her to incorporate and provide care in all areas of medicine for all ages. Mandy, a Delta native, enjoys building relationships of trust with each of her patients and is committed to providing excellent care to each and every one of her patients.
When she is out of the office, Mandy loves to travel and experience other cultures in their native environments. She loves yard work, beaches, supporting and cheering on each of her 8 children, spending time with her family, and working on home improvement projects.
A provider's patient satisfaction rating is determined by patient responses to questions from the Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey, administered by NRC Health.

CAHPS surveys are funded and overseen by the Agency for Healthcare Research and Quality (AHRQ) within the United States Department of Health and Human Services (HHS).
Patients receive the nine-question survey via email or phone call, and we publish provider ratings on Revere Health's website once he or she has at least 30 completed surveys.
