Kelly Ence, MD Departure Letter

Dear Patients and Friends,

I will be leaving my full-time practice at Revere Health as of December 31, 2022, after 18 wonderful years. I have also enjoyed practicing for 4 years in the U.S. Air Force and 11 years in Utah County prior to joining Revere Health. I will pursue some temporary work as I move toward full retirement.

It has been a pleasure to take care of you over the years, and I hope you will continue to come to Revere Health and visit any one of my talented colleagues for your health needs. I have full confidence in the abilities of Dr. Derek Vanhille and James Crowley, PA, to care for you.

Your medical records will remain at Revere Health, but if you choose to seek care outside of Revere Health you may fill out an authorization form to transfer your records to a new provider. You can also use our FollowMyHealth portal to download a copy of your records. Call 801.429.8062 for assistance.

Thank you for the opportunities to learn and grow with you as your physician. I have always strived to advocate for my patients and foster relationships built on trust and mutual understanding. I will miss caring for you—I hope I was able to make a positive difference in your life during our time together.
