Application for Medical Certification
*** Prior to your exam please go to medexpress.faa.gov. At this website, you will need to create an account and then proceed with an Application for Medical Certification. When you complete the data entry and submit you will receive a confirmation number. You will need to bring the confirmation number with you to your exam.

There are medical conditions and medications that require status reports from your treating provider in order for you to be medically certified. You will need to provide this at the time of your exam. When you schedule your appointment we will provide instructions regarding these reports.
