Bio	Jake Church, PA-C earned a Bachelor of Science in Exercise Science with a minor in Spanish at Brigham Young University. He received his Master of Physician Assistant Studies at Penn State College of Medicine Physician Assistant Program. Jake speaks fluent English and Spanish. Jake has been interested in medicine for as long as he can remember. He was drawn to Revere Health because of the excellent reputation of its providers, and he hopes to continue learning and growing as a provider himself. Jake will strive to meet the needs of his patients in a way that aligns with Revere Health’s mission of value-based care. While Jake is not taking care of patients, he loves to watch and play sports. He also enjoys card games, board games, mountain biking, and traveling.
