2015 HealthInsights Quality Award—Revere Health

Four of Revere Health’s internal medicine locations earned quality awards in 2015: Orem, Provo, American Fork, and Payson. HealthInsight, a nonprofit, community-based healthcare collaborative, sponsors the award.

To receive this award, practices must meet a set of criteria related to innovation, patient engagement, and quality initiatives. Though we have always focused on quality and patient-centered care, this year as a part of our rebrand, we incorporated these as a part of our core values.

Our internal medicine clinics are known for their innovations in patient care. In the past year, our internal medicine groups have worked to reduce the amount of readmissions to the hospital in our high-risk patients. Part of our plan has included finding those people who were most likely to go back to the hospital after a planned or unplanned visit, and then implement a strategy to engage patients in their post-hospital recovery.

With this plan in mind, our providers have been working to make sure that once a patient leaves the hospital, they are in contact with their provider and have the education they need to get on the road to recovery without readmission.
