My Dear Patients and Friends,
It has been my great pleasure to care for you over the last several years, but I am writing to let you know that May 19, 2023, will be my last day at Revere Health. If you have an appointment with me after May 19, please call the office at 385-203-1240 and use option #1 to reschedule your appointment.
I have truly enjoyed caring for you at Provo Neurology, and I want to thank you for choosing me as your trusted care provider. I hope you will continue to come to our clinic and visit any one of my talented colleagues for your Neurology care needs. Douglas Langford, MD, and Evangeline Lindorf, APRN, will be happy to take care of you.
Your medical records will remain at Revere Health, but if you choose to seek care elsewhere, you may fill out an authorization form to transfer your records to a new provider. Call 801.429.8062 for assistance. You can also use the FollowMyHealth app, our online patient portal, to download a copy of your medical records.
My goal as a Revere Health provider was to deliver competent and dignified patient care, and I hope I was able to provide that for you. I thank you for the positive impact you made on my life, and I wish you the best as you continue your journey toward improving your health and overall well-being.
