Marvin Allen, MD, Takes Leave of Absence to Serve Church

Dear Valued Patient,

It is with mixed emotions that I am writing to inform you that my wife, Lori, and I have received a call to serve as mission leaders for the Church of Jesus Christ of Latter-Day Saints in Auckland, New Zealand for a period of three years. As a result, my last day at the clinic will be April 26, 2023. I am planning on returning to my practice in the fall of 2026.

If you have a preference for a particular physician you’d like to see, please let my staff know when you are contacted to schedule your next appointment. Additional providers are listed below.

Your medical records will remain at Revere Health, but if you choose to seek care outside of Revere Health you may fill out an authorization form to transfer your records to a new provider. You can also use our FollowMyHealth portal to download a copy of your records. Call 801.429.8062 for assistance.
